What the record shows
Gross pay, taxes withheld, net pay, and the year-to-date total for one period. The figures have to match the payroll that was run.
Do not change the wage to make the year look larger.
What this form is not
It is not a credit application and it is not a letter from a payroll company. If someone asks for the stub the employer issued, ask the employer or the payroll provider for that copy.
